$199 Teeth Whitening for Life Program

At Serenity Dental, we are pleased to offer $199 Whitening for Life Program. We desire our patients to have beautiful and healthy smiles that they can be proud of. We believe that this program will help our patients maintain beautiful and healthy smiles through regular professional care.

How It Works?

• You pay an enrolment fee of $199
• You will receive custom made whitening trays, 3 tubes of whitening gel, and a case for the trays
• You will receive One FREE tube of whitening gel at Recall Visit for the rest of your life*

Guidelines for New Patients:

• Each patient must be at least 16 years old, not pregnant or nursing
• Each new patient must complete a Comprehensive New Patient Exam, appropriate x-rays, cleaning and follow through with any necessary treatment plans like filling cavities, managing gum disease, and other deteriorative issues (services typically covered by most insurance companies). Healthy gums and teeth are required to receive professional whitening service; otherwise whitening could be detrimental to your oral health
• You should not have any outstanding bills or insurance claims with Serenity Dental. All outstanding dues should be cleared within 30 days
• As a courtesy, you must provide 48 hour notice before postponing or changing your appointment.

Guidelines for Existing Patients:

• Must be at least 16 years of age, not pregnant or nursing
• Must comply with minimum required dental care as treatment planned
• Must have at least six-month patient history without any missed appointment
• Must provide at least 48 hour notice if you need to postpone or change your appointment
• Must not have any outstanding bills with Serenity Dental

Guidelines for Lifetime Maintenance

• You should comply with your hygiene schedule of at least two visits a year at approximately 6 month intervals. Your hygiene visits should not be more than 20 days  “overdue”.
• You should not have any outstanding bills or insurance claims with Serenity Dental. All outstanding dues should be cleared within 30 days.
• As a courtesy, you must provide 48 hour notice before postponing or changing your appointment.
• Our doctors reserve the right to modify or cancel this program for the patient’s well-being, if future research modifies the guidelines on dental teeth whitening.
• If you desire a higher level of brightness or have teeth that are more prone to darken, can purchase additional whitening gel (above their free annual tubes) for $15/tube. **
• Trays will be evaluated every three years for the fit. Replacement, if needed, will be at patient’s cost.

Please note that lost and broken trays will be replaced at patients cost. The replacement cost will be $75 ($50 if you still have your models). **

*The tubes at no-charge are the number required to maintain white smile for the “average” dental patient. Additional tubes are available to purchase for those patients desiring even brighter teeth or for those who have naturally very dark tones in their teeth

**The cost of replacement trays and whitening gel will be evaluated annually and adjusted to reflect changes in material and operating costs.

Should any of the rules fail to be met, you will immediately be disqualified from the program and provided the opportunity to re-enrol for a $50 re-activation fee.

If at any time the price of whitening solution reaches unreasonable cost levels, Serenity Dental reserves the right to modify or cancel this program if the additional costs are outside our promotional budget. We don’t anticipate this occurring and appreciate your patronage and referrals which help us maintain a vital and prosperous office!
